BACKGROUND: We present an interesting and unusual case of a gunshot wound to the head with severe brain injury. The patient survived and presented only minor neurological deficits upon discharge. METHODS: An early and aggressive surgical and perioperative management. RESULTS: Our patient confirms that the outcome as measured by the Glasgow Outcome Scale strongly correlates with the condition upon admission, as determined by the Glasgow Coma Scale. CONCLUSIONS: Intracranial gunshot wounds need to be rapidly and thoroughly evaluated in a rational and systematic manner to minimize mortality and long-term morbidity.

GRUNDLAGEN: Wir präsentieren einen interessanten und ungewöhnlichen Fall eines Patienten mit einer Kopfschussverletzung und schwerem Gehirntrauma. Der Patient überlebte das Trauma und zeigte bei seiner Entlassung lediglich geringe neurologische Ausfälle. METHODIK: Ein promptes und aggressives chirurgisches und perioperatives Management. ERGEBNISSE: Unser Patient bestätigt, dass das Ergebnis, gemessen am Glasgow Outcome Scale, mit dem Aufnahmestatus, gemessen am Glasgow Coma Scale, eindeutig korreliert. SCHLUSSFOLGERUNGEN: Kopfschüsse müssen rasch, systematisch und vollständig evaluiert und therapiert werden, um Mortalität und Langzeit-Morbidität zu verringern.
