De Garengeot’s hernia—a rare finding occurring mostly in women—is defined by the presence of the vermiform appendix within
the sac of a femoral hernia. The incidence of appendicitis is rarer still, with less than a 100 cases reported to date. We
present a unique case of an 84-year-old male patient with perforated appendiceal diverticulitis within a De Garengeot’s hernia
causing abscess and necrotizing infection of the overlying soft tissues.
