The experience of the surgeons at the Clinique du Parc to Lyon, based on over 2,000 implanted artificial ligaments for replacement of the anterior or posterior cruciate ligament of the knee, dates back over 10 years to 1980. Today, this experience makes, it is possible to document the indications of this type of surgical technic which is still the subject of much controversy and dispute. In order to improve the analysis of clinical results, over the last year or so, the authors have computerized data and systematically reexamined all patients operated on and not lost to follow-up. The results obtained for the 325 ABC ligaments thus reexamined are presented here and support the following indications: recently-developed laxity of the anterior ligaments, in patients over 27 years of age, whether the impairment be isolated as in the athlete, or complex, most often inserted arthroscopically; certain types of chronic laxity of the anterior ligament. Contraindications are documented. In cases of chronic instability of the posterior ligament combining rupture of the posterior cruciate ligament with postero-lateral impairment, the artificial ligament is used in combination with valgus osteotomy.

Remontant à 1980, il y a plus de dix ans, portant sur plus de 2 000 ligaments artificiels implantés pour remplacer le ligament croisé antérieur ou le croisé postérieur du genou, l'expérience des chirurgiens de la Clinique du Parc à Lyon permet aujourd'hui de préciser les indications de ce type de technique chirurgicale dont l'utilisation suscite toujours controverses et discussions. Pour améliorer l'analyse des résultats cliniques, les auteurs ont entrepris de mettre en informatique et de revoir systématiquement depuis plus d'un an la majorité des malades opérés et retrouvés. Les résultats de 325 ligaments ABC ainsi revus sont présentés et servent de support aux indications : dans les laxités antérieures fraîches, volontiers après 27 ans, que la lésion soit isolée chez le sportif ou complexe, le plus souvent sous arthroscopie; dans certaines laxités antérieures chroniques. Les contre-indications sont précisées. Dans les laxités postérieures chroniques associant une rupture du ligament croisé postérieur à une lésion postéro-externe, le ligament artificiel est utilisé, associé à une ostéotomie de valgisation.
