Evidence-based models and frameworks have been introduced to support diabetes self-management education and support. This
article presents various frameworks and models and describes their use in support of diabetes education at the patient–educator,
the practice environment, and the systems/policy/environmental level. The text and tables present various models and specific
recommendations and examples for educators to use at every level. Crosscutting concepts are that models that support diabetes
education at all levels include an assessment, goal setting, problem solving, and systematic follow-up; an ongoing process;
and community and primary care approaches that attempt to provide outreach and sustainable programs. Tested frameworks and
models serve to support the provision of diabetes education at all levels and should be used in continued implementation and
evaluation.
